The PT also provides staff training, supervises PTAs, and ensures accurate documentation.</p> <p>Key Responsibilities</p> <ul> <li> <p>Perform evaluations within 24 hours or 1 business day of physician referral</p> </li><li> <p>Develop treatment plans and obtain physician approval</p> </li><li> <p>Provide direct treatment according to the care plan</p> </li><li> <p>Supervise Physical Therapist Assistants (PTAs) per state practice act</p> </li><li> <p>Document evaluations, daily notes, progress notes, and recertifications in compliance with PT Board and payer requirements</p> </li><li> <p>Participate in care conferences, utilization reviews, and interdisciplinary meetings</p> </li><li> <p>Provide caregiver and family training for safe discharge planning</p> </li><li> <p>Recommend necessary DME (braces, walkers, canes, grab bars, etc.)</p> </li><li> <p>Assist with Restorative Aide training</p> </li><li> <p>Maintain equipment and report any issues</p> </li><li> <p>Support facility in-service programs and staff education</p> </li></ul> <p>Qualifications</p> <ul> <li> <p>Bachelor's, Master's, or Doctorate in Physical Therapy</p> </li><li> <p>Active California PT license in good standing</p> </li><li> <p>Strong communication skills and the ability to collaborate with residents, families, and staff</p> </li><li> <p>Proficiency with EMR systems (Casamba, PointClickCare preferred)</p> </li></ul> <p>If you're a dedicated Physical Therapist looking to grow your career in a supportive, resident-centered environment, apply today to join Vineyard Post Acute!</p> <p>We are an equal opportunity employer.
