<ul> <li>Area of Interest: Nursing </li><li>FTE/Hours per pay period: 0.7 </li><li>Department: Case Management </li><li>Shift: 0800-1630, there may be a few weekends and 1-2 holidays/year </li><li>Job ID: 181898 </li></ul> <p>Overview</p> <p>RN Case Manager</p> <p>Marshalltown Hospital</p> <p>Shift: Part-Time | 28 Hours per Week | 8:00 AM-4:30 PM</p> <p>Includes occasional weekend coverage and approximately 1-2 holidays per year.</p> <p>As an RN Case Manager, you will play an integral role in coordinating high-quality, cost-effective patient care across the continuum of care. You'll collaborate closely with providers, nursing staff, social services, patients, and families to support safe discharge planning, appropriate utilization of resources, and positive patient outcomes.</p> <p>This role focuses on care coordination, utilization review, and interdisciplinary collaboration to ensure patients receive the appropriate level of care while helping reduce avoidable admissions and barriers to discharge.
