You'll collaborate closely with providers, nursing staff, social services, patients, and families to support safe discharge planning, appropriate utilization of resources, and positive patient outcomes.</p> <p>This role focuses on care coordination, utilization review, and interdisciplinary collaboration to ensure patients receive the appropriate level of care while helping reduce avoidable admissions and barriers to discharge.
