<p>Employment Type:</p> <p>Full time</p> <p>Shift:</p> <p>Description:</p> <p>Hospice Certified Nurse Assistant/Home Health Aide, (Enfield Region)</p> <p>Location: Enfield Region</p> <p>Status: Full Time</p> <p>Bonus: Sign On Bonus</p> <p>Trinity Health Of New England At Home serves patients and their loved ones by providing a variety of in-home services including but not limited to nursing, social work and physical/occupational/speech therapies.</p> <p>We are committed to providing compassionate, exceptional care where people are most comfortable: at home!</p> <p>A Catholic, Mission-driven, non-profit organization, we are affiliated with Johnson Memorial Hospital of the Trinity Health Of New England health system and are part of Trinity Health At Home, a National Health Ministry of Trinity Health, one of the largest Catholic healthcare systems in the United States.</p> <p>Position Purpose:</p> <p>Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.</p> <p>Position Details:</p> <p>Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year.</p> <p>Coverage Region: Enfield</p> <p>Bonus: Position qualifies for a $5,000 sign-on bonus paid out over two years.</p> <p>NOTE: This position is 100% home care providing services in patients' private homes.</p> <p>What you will do:</p> <ul> <li> <p>Follows care plan developed by RN.</p> </li><li> <p>Reports client changes in condition to RN, as described on care plan.</p> </li><li> <p>Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.</p> </li><li> <p>Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.</p> </li><li> <p>Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.</p> </li><li> <p>Performs range of motion and other simple procedures as an extension of therapy services as assigned.</p> </li><li> <p>Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record.
