Join us in making a positive impact on our residents' lives!</p> <p>Job Summary: The Physical Therapist Assistant shall implement the plan of care as established by the RPT and the physician in a professional, competent manner, upholding excellence with quality of care.</p> <p>Responsibilities/Requirements:</p> <ul> <li> <p>Screen all residents in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Physical Therapy intervention is indicated.</p> </li><li> <p>Carry out treatment program according to plan of care in consultation with Registered Physical Therapist</p> </li><li> <p>Identify the need for modification of treatment plan and consult with Registered Physical Therapist.</p> </li><li> <p>Monitor and report resident performance and progress.</p> </li><li> <p>Assist Registered Physical Therapist in re-assessment of patients. Assist in determining need for continued services or discontinuation of services based on patient progress.</p> </li><li> <p>Assist Registered Physical Therapist in discharge planning.</p> </li><li> <p>Supervise Rehab Therapy Aides.</p> </li><li> <p>Educate staff, family and care givers as indicated.</p> </li><li> <p>Identify needs for program development and implement programs according to company policies and procedures.</p> </li><li> <p>Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy.</p> </li><li> <p>Complete all required documentation according to guidelines specified in Documentation Manual.</p> </li><li> <p>Coordinate in-services for facility staff as needed is determined, to ensure safe and effective carryover of Maintenance/Restorative Programs and overall call of the residents.</p> </li><li> <p>Attend all facility required meetings.</p> </li><li> <p>Promote profession within facility, including coordinating special events during National Physical Therapy Month.</p> </li><li> <p>Attend Continuing Education courses specific to current position.</p> </li><li> <p>Always carry out all job responsibilities in a professional manner.</p> </li></ul> <p>Qualifications</p> <p>Qualifications:</p> <ul> <li>Associate degree in Physical Therapy Assistant program from an accredited institution. </li><li>Certification and licensure requirements as established by state and national board associations. </li><li>Active state license required. </li><li>Excellent organizational and time management skills </li><li>Excellent written and verbal communication skills </li><li>Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines. </li><li>Completion of all required screenings, certifications, and licensure.
