Paid at 90 days of employment. Must have worked an average of 30 hours a week in the prior 90 days </li><li>Part Time - $1,000.00. Paid at 90 days of employment. Must have worked an average of 20 hours a week in the prior 90 days </li></ul> <p>Schedule:</p> <ul> <li>Full-Time 32 visits or more / Part-Time 29 visits or less </li></ul> <p>Role Overview:</p> <p>We are seeking motivated RN's with experience in Home Health Skilled Nursing Visits to join our team in Broomfield/Thornton. In this role, you will visit our clients in their home and help with skills needing to be assessed or managed and offering education.</p> <p>Key Responsibilities:</p> <ul> <li>Provide skilled nursing care to patients in their homes </li><li>Work independently to assess patient needs, develop care plans, and implement appropriate interventions </li><li>Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes </li><li>Communicate effectively with patients, families, team members, and other healthcare professionals to ensure coordinated care </li><li>Adhere to all regulatory requirements and company policies and procedures </li></ul>
