This person provides case management services to individuals of all ages with a primary diagnosis of intellectual or developmental disability. This includes assessing service needs; developing plans for access to services; liaison with client, families, programs, and service providers; ongoing monitoring or client service needs; advocacy; and consultation and education to clients, families and the community. This is a wonderful opportunity to join a group built on our company values: "It's Our Responsibility to Care."</p> <p>4,500 Sign on Bonus</p> <p>Starting Salary Rates (Dependent Upon Experience):</p> <p>Case Manager (with High School Diploma): $39,000</p> <p>Case Manager (with Bachelor's Degree): $42,900</p> <p>Education/Experience Required:</p> <ul> <li>High school diploma with at least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or intellectual disabilities is required. </li><li>Bachelor's degree in psychology, counseling, social work, special education, or other Virginia DMAS approved related human services field is preferred. </li><li>Experience providing professional supports to individuals with intellectual disabilities is preferred. </li></ul> <p>Additional compensation will be considered for any new employees who speak multiple languages.</p>
