We share the commitment to delivering a coordinated and comprehensive experience across all locations, ensuring the appropriate level of care is available to every patient across our care delivery sites.</p> <p>Located at 10 Members Way in Dover, NH, our Diabetes Services offers a comprehensive program designed to support individuals across all stages of diabetes management, including type 1, type 2, prediabetes, and gestational diabetes. The hospital's multidisciplinary team comprises board-certified endocrinologists, certified diabetes educators, registered dietitians, and specialized nursing staff who collaborate to deliver personalized care plans. Services encompass individualized and group education sessions focusing on blood glucose monitoring, nutritional counseling, exercise programs, and medication management. In addition to clinical services, the hospital provides community outreach through educational classes, support groups, and seasonal camps for children with diabetes. The program emphasizes a patient-centered approach, aiming to empower individuals with the knowledge and tools necessary for effective self-management and improved quality of life.</p> <p>We are seeking a part-time Diabetes Educator to join our Diabetes Services team! The desired schedule is Tuesdays, Wednesday and Thursdays from 8:30am to 5:00pm. This Diabetes Educator will float to both our Dover location at 10 Members Way and our Primary Care of Rochester location.</p> <p>Job Summary</p> <p>Summary</p> <p>Responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. This role involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs.</p> <p>Does this position require Patient Care?</p> <p>Yes</p> <p>Essential Functions</p> <ul> <li>Conduct assessments to determine the educational needs and readiness of patients to learn about diabetes management. </li><li>Develop and implement individualized education plans addressing diabetes self-management, including medication management, nutrition, physical activity, and blood glucose monitoring. </li><li>Facilitate group education classes on diabetes management, prevention, and lifestyle modifications. </li><li>Create educational materials and resources to enhance patient understanding and engagement. </li><li>Provide ongoing support and counseling to patients and their families, addressing the emotional and psychological aspects of living with diabetes. </li><li>Encourage patient adherence to treatment plans and empower them to actively participate in their health. </li></ul> <p>Qualifications</p> <p>Education Associate's Degree Related Field of Study required and Bachelor's Degree Related Field of Study preferred and Master's Degree Related Field of Study preferred Can this role accept experience in lieu of a degree?
