<p>POSITION SUMMARY:</p> <p>This is an outreach/environmental/community services position. This person may work under the regular supervision of a higher classification. As the person gains work experience within the Agency, he/she will assume a selected caseload through which an increasing knowledge of Agency policies and procedures may be acquired. Duties may change depending on program needs.</p> <p>The individual is expected to embrace the co-occurring treatment philosophy which integrates a strength based, child/family centered, trauma informed, culturally competent, and recovery oriented philosophy. The staff will carry a selected range of cases through which an increasing knowledge of best practice principles and methods, and knowledge of community resources is demonstrated.</p> <p>GENERAL DUTIES/ RESPONSIBILITIES:</p> <ul> <li>Interprets Agency services to clients and community. </li><li>Written case record maintained for each family served and contains information required. </li><li>Standardized case closing form completed and submitted to DCF case worker or community support worker no later than 10 days after end of service. </li><li>Complete standard Parenting Support Services checklist for each Session with family. </li><li>Complete Parenting Support Services baseline assessments and Family Service Plan (FSP) within first 3 visits. </li><li>Initial in home (or Parent Preferred location) visit as soon as possible but at least within 15 business days of receipt of referral. </li><li>Assist families with light case management. </li><li>Maintain accurate and up to date data in data systems. </li><li>May conduct presentations in the community to promote the program and recruit clients. </li><li>Successfully engage clients in services as demonstrated by client service plan completion rates, maintained or increased client functioning and client completion of service plan objectives. </li><li>May formulate mutually agreed-on and measurable service plan goals and objectives. </li><li>Perform 100% of assigned productivity expectation. </li><li>Encourage increased independence and recovery of clients. </li><li>Understand and follow agency policies and procedures. </li><li>Participate in ongoing supervision, consultation, staff meetings, and appropriate training. </li><li>May function as team member with others. </li><li>Identifies community needs. </li><li>Learns community resources. </li><li>Provides advocacy services. </li><li>Carries volume of work within range established by Agency. </li><li>Accept and carry out other related duties upon request </li><li>PQI Functions: </li><li>Ensure the consistent accurate collection of the data </li><li>Identify patterns and trends for program </li><li>Use results of data to inform supervisor of trends that may impact services </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>Bachelor's degree or Associates Degree working towards a Bachelor's degree preferably in a human service field with knowledge of Community Resources. </li><li>In addition to the above qualifications, specialized knowledge and experience not necessarily related to formal education or training (e.g. knowledge and understanding of another language, a sub-culture) which equips him/her to provide services to a particular segment of the Agency's clientele may be required. </li><li>Triple P Level 4 certification </li><li>Triple P level 4 training in either the 0-12 or teen level with willingness to be trained in the other level </li><li>Circle of Security certification (COS) </li><li>Bilingual (English/Spanish) preferred. </li></ul> <p>COMPETENCIES:</p> <ul> <li>Experience providing Triple P / COS interventions preferred </li><li>Experience/comfort in providing help to families experiencing behavioral/emotional difficulties with recent onset </li><li>Promotes family empowerment and strengths by initiating and maintaining respectful, functional relationships with parents and their children while maintaining appropriate boundaries </li><li>Demonstrated initiative, self-direction, flexibility, leadership and decision making skills </li><li>Demonstrated ability to work cooperatively as a member of a team and with other community agencies as well as independently with minimal supervision. </li><li>Excellent written and oral communication skills </li><li>Work constructively and contribute to a harmonious and effective work environment </li><li>Handle work in a professional manner with tact, diplomacy and confidentiality </li><li>Solid problem-solving abilities within the Triple P/ COS scope of practice; ability to set priorities and manage conflicting demands </li><li>Strong time management and organizational skills </li><li>Excellent interpersonal, communication and public relations skills with capacity to relate to clients of different experiences and needs </li><li>Careful attention to detail </li><li>Skills to maintain client records and database </li><li>Flexible work hours to ensure access to the service </li></ul>
