This full-time (40 hours/week) role consists of working afternoon and evenings Monday - Friday typically from 10 a.m. to 6 p.m.</p> <p>What you will do:</p> <p>Empower people to live their best life by providing supports in the home and out in the community for our CONNECT Program families</p> <ul> <li>Provide one-on-one support to six to eight adults living independently, and act as an advocate in all aspects of life </li><li>Assisting with various life skills including, but not limited to, everyday household needs, community and social access, finances, transportation, and household responsibilities </li><li>Create and support community based relationships for individuals in the CONNECT program </li><li>Teach technical skills, use of devices, and apps to novice users with and without disabilities </li><li>Communicate with individual's family members, case managers, job supports, etc. </li><li>Document progress towards outcomes per funding guidelines </li></ul> <p>What you can expect from us:</p> <ul> <li>We'll celebrate and empower your unique gifts and contributions. </li><li>We'll help you help others.
