Discover your opportunities at Yale!</p> <p>Hourly Range</p> <p>27.74</p> <p>Overview</p> <p>The Transitions Clinic Hub Community Health Worker (CHW) will provide outreach, counseling, navigational, and case management services to people who are preparing to release from incarceration and those who are recently released from prison with chronic diseases including those affected by substance use (SU) and/or mental health (MH) issues. The Community Health Worker will staff the newly funded TC-HUB, a referral infrastructure for those returning from incarceration. The TC-HUB is housed at the SEICHE Center for Health and Justice at Yale School of Medicine. The Community Health Worker will work from within the context of a patient-centered, team approach and in continuous partnership with community health clinics, halfway houses, and the Department of Corrections to help navigate the healthcare system and address health related social needs.</p> <p>The CHW will operate in a supportive role within an interdisciplinary health care team utilizing an integrated care and treatment model and provide outreach, case management and navigational services to individuals who are pending release and recently released from prison, specifically, those with chronic medical, psychiatric and substance use conditions. The CHW will be supervised by the TC-HUB nurse manager and work to ensure a smooth care transition from prison to the community, operating in the three months prior to patient's release from prison and the immediate post release period. The CHW will establish referrals, set up initial appointments in the community, and address health related social needs with and for the patient.</p> <p>Required Skills and Abilities</p> <ol> <li> <p>Willingness to attend and complete Community Health Worker Training within first 180 days of employment and certification which is required by the state.</p> </li><li> <p>Valid and verifiable Connecticut driver's license and excellent driving record and reliable transportation to get to correctional facilities. Willingness to work in various environments, including prison/jail settings, street outreach, halfway houses, home visits, patient residence, homeless encampments and homeless shelters.</p> </li><li> <p>Comfortable working with culturally diverse populations, LGTBQ, formerly and currently incarcerated individuals including persons with a sex offense, and those with mental health and substance use issues. Demonstrated experience in the reentry field.</p> </li><li> <p>Excellent verbal and written communication skills. Ability to provide documentation of interactions and experiences with participants including basic computer skills and knowledge of Microsoft Word. Basic internet skills.</p> </li><li> <p>Excellent organization, follow through, and ability to juggle multiple priorities in fast-paced environment with multiple collaborators. Flexible and able to take initiative. Demonstrated excellent attendance and reliability. Able to work flexible job hours (including some evenings and weekends).</p> </li></ol> <p>Preferred Skills and Abilities</p> <p>Previous work experience as a Community Health Worker. Community Health Worker Certification. Bilingual English/Spanish.</p> <p>Principal Responsibilities</p> <ol> <li>Researches and collects data through site or home visit intakes; library research; structured interviews; or through other means for designated research assignments. 2. Records and compiles information related to research data. Codes data accordingly to research specifications. Uses a computer terminal to input and retrieve data and to generate reports. 3. Processes and summarizes data using scientific or statistical techniques. Assists in data interpretation and analyses. Reports on status of research activities. 4. Recruits study participants and ensures that subject recruitment and follow-up are completed per protocol procedures. 5. Orders and maintains inventory of supplies. May assist in designing, developing, and modifying research experiments, procedures, or survey instruments. May assist research and support staff. 6. Performs additional functions incidental to research activities. Required Education and Experience Six years of related work experience, four of them in the same job family at the next lower level, and high school level education; or four years of related work experience and an Associate's degree; or little or no work experience and a Bachelor's degree in a related field; or an equivalent combination of experience and education. </li></ol> <p>Background Check Requirements</p> <p>All candidates for employment will be subject to pre-employment background screening for this position, which may include motor vehicle, DOT certification, drug testing and credit checks based on the position description and job requirements. Healthcare workers (HCW) are defined as university employees working a healthcare setting who have the potential for direct or indirect exposure to patients, human research subjects or infectious materials including body substance, contaminated medical supplies, devices and equipment, surfaces, or air.
