Our employees make all the difference in our success!</p> <p>Schedule-Monday-Friday-8am-5pm</p> <p>Benefits Day 1</p> <p>Mileage Reimbursement</p> <p>Key Responsibilities:</p> <ul> <li>Communicate successfully with peers, supervisors, public, and staff members in a manner that represents Hope's culture of a collaborative, positive work environment. </li><li>Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants' baseline status to appropriate staff on a daily basis. </li><li>Involved in the development and implementation of quality assessment and performance improvement (QAPI) activities. </li><li>Provide regular updates to team on participants' status in therapy. </li><li>Coordinate services according to the plan of care with external vendors, ensuring appropriateness of treatment plans and interventions. </li><li>Assess the need for adaptive equipment and durable medical equipment (DME); order and facilitate authorization of such equipment; assess, recommend, and provide follow-through therapy for specialized shoes. </li><li>Participate in meetings as indicated (morning, team, family, nursing facility, assisted living facility, and hospital). </li><li>Provide treatment as indicated. </li><li>Utilize appropriate safety measures when providing care. </li><li>Develop special programs according to participant needs. </li><li>Educate staff, participants, and family on services available through therapy staff. </li><li>Develop, instruct, and supervise other staff, and/or family members on therapy program to be carried out by individuals other than a physical therapist. </li><li>Provide orientation and training in regards to body mechanics and lift equipment to staff as requested. </li><li>Communicate observed staffing needs/changes and further program development needs to supervisor. </li><li>Complete documentation as required. </li><li>Adjust schedule as needed to meet the needs of PACE program participants. </li><li>Maintain confidentiality regarding participant, staff, contractor, and organizational information. </li></ul> <p>Knowledge & Skills:</p> <ul> <li>Excellent communication; via phone, email, and in person. </li><li>Attention to detail & ability to handle confidential information following HIPPA guidelines. </li><li>Must be flexible to work in other locations if needed. </li></ul> <p>Required qualifications:</p> <ul> <li>Degree in Physical or Occupational Therapy required. </li><li>Licensure required. </li><li>One year elder care work experience with the frail elderly required. </li><li>Management experience preferred. </li><li>Valid Florida Drivers' License with good record </li><li>Automobile Insurance </li><li>Clear level 2 background check </li><li>Active BLS for healthcare professionals from the American Heart Association or Red Cross; Current employees must obtain by January 1, 2025. </li></ul> <p>This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.</p>
