<p>The VA Northeast Ohio Healthcare System is actively recruiting for a Social Worker. The incumbent is a social worker within Social Work Service and may provide coverage to all areas of the medical center in general social work practice.</p> <p>The Social Worker duties include but are not limited to:</p> <ul> <li>Identifies high-risk patients and provides case management services. </li><li>Performs High Risk Screening Assessments on all newly hospitalized veterans within two business days of admission to identify psychosocial factors that may affect medical care or discharge planning. </li><li>Establishes and maintains effective therapeutic relationships with veterans and their families, and recognizes strengths, weaknesses or other family dynamics that can either support or impede care or D/C plans. </li><li>Performs case finding functions and outreach, identifying and screening patients for psychosocial needs. </li><li>Develops discharge plans in collaboration with the veteran/family and with the interdisciplinary treatment team. </li><li>Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. </li><li>Works with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. </li><li>Maintains general knowledge of veterans' benefits and services relating to special programs (e.g. Caregiver Support), service-connected compensation, non-service-connected pension.
