<p>Job Description: The Nurse Care Manager provides support to targeted patients by coordinating care with patient hospital observation and inpatient discharges.# The Nurse Care Manager will ensure the provision of quality healthcare services in a cost-efficient manner through daily planning, coordination, education, and monitoring of care.# The Nurse Care Manager will track gaps in care and relevant outcomes measures for the patients to ensure complete and effective care.# This is a remote position, must live local. # ##To qualify you must have: Current RN license for the State of Indiana required.
