<p>Job Description:</p> <p>I. Position Summary</p> <p>The Registered Nurse (RN) and Licensed Practical Nurse (LPN) work collaboratively to deliver high-quality, patient-centered nursing care using the nursing process of assessment, planning, implementation, and evaluation. They are integral members of the multidisciplinary clinical team, committed to the mission and philosophy of Hebrew SeniorLife (HSL), ensuring patients' dignity, autonomy, and quality of life.</p> <p>RNs maintain accountability for comprehensive assessments and clinical oversight, while LPNs contribute under the supervision of an RN, focusing on ongoing care delivery and supporting care plans. Both roles actively support culture change initiatives and serve as role models and mentors within their units.</p> <p>II. Core Competencies</p> <ul> <li> <p>Demonstrates empathy and compassion in all patient and family interactions</p> </li><li> <p>Supports patient independence and choice in care decisions</p> </li><li> <p>Strong communication skills and ability to foster interdisciplinary teamwork</p> </li><li> <p>Commitment to excellence and delivery of high-quality, evidence-based care</p> </li><li> <p>Accountability and ownership in clinical practice and teamwork</p> </li><li> <p>Strong critical thinking, organizational, and problem-solving abilities</p> </li><li> <p>Ability to coach and mentor peers, PCAs, and other staff members</p> </li></ul> <p>III. Position Responsibilities</p> <p>Shared Responsibilities (RN & LPN):</p> <p>Deliver nursing care consistent with professional standards, policies, and HSL core competencies</p> <p>Collaborate with interdisciplinary team (MDs, NPs, social workers, therapists, dietitians, etc.) to meet patient needs</p> <p>Implement evidence-based interventions to achieve optimal outcomes</p> <p>Participate in care planning, promoting autonomy and individual choice</p> <p>Document accurately in electronic medical records</p> <p>Identify and utilize appropriate resources to ensure safe, effective care</p> <p>Maintain compliance with all policies, regulations, and HIPAA</p> <p>Participate in quality assurance, performance improvement initiatives, and shared governance</p> <p>Respond to and report safety concerns or incidents</p> <p>Actively engage in unit activities and assist with tasks (e.g., meal service)</p> <p>Serve as role models and mentors to PCAs and other clinical staff</p> <p>RN-Specific Responsibilities:</p> <p>Conduct comprehensive assessments and lead care planning</p> <p>Provide clinical oversight to LPNs and PCAs</p> <p>Revise care plans based on assessment data</p> <p>Co-sign LPN assessments and initial care plans</p> <p>Initiate incident, DPH, and concern resolution reports</p> <p>Lead rounds (wound care, physician visits)</p> <p>May assume charge nurse responsibilities, including:</p> <ul> <li> <p>Delegating assignments and supervising shift operations</p> </li><li> <p>Completing and overseeing admissions and discharges</p> </li><li> <p>Contributing to staff performance evaluations and education</p> </li><li> <p>Developing patient care assignments and staffing schedules</p> </li></ul> <p>LPN-Specific Responsibilities:</p> <p>Support RN-led assessments and contribute to care implementation</p> <p>Supervise and delegate to PCAs</p> <p>Ensure care plan fidelity and update documentation under RN oversight</p> <p>Assist with reports (DPH, incidents) as directed</p> <p>May assume charge nurse duties under RN supervision, including:</p> <ul> <li> <p>Providing shift reports</p> </li><li> <p>Monitoring documentation</p> </li><li> <p>Attending rounds</p> </li><li> <p>Contributing to care assignments and staff guidance</p> </li></ul> <p>IV. Qualifications</p> <p>For Both RN and LPN:</p> <ul> <li> <p>Graduate of an accredited nursing school</p> </li><li> <p>Valid Massachusetts nursing license in good standing (RN or LPN)</p> </li><li> <p>Previous clinical experience preferred</p> </li><li> <p>Strong interpersonal, communication, and leadership skills</p> </li><li> <p>Flexibility, professionalism, and adaptability to changing priorities</p> </li><li> <p>Computer literacy and proficiency with electronic medical records</p> </li><li> <p>Excellent English communication skills (written and verbal)</p> </li></ul> <p>RN Additional Preferred Qualification:</p> <ul> <li>Bachelor of Science in Nursing (BSN) preferred </li></ul> <p>Remote Type</p> <p>On-site</p> <p>Hebrew SeniorLife is an equal opportunity employer.
