<p>This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.</p> <p>The position is located in the Supportive Recovery Services (Substance Use Disorder clinic) of the Mental Health Patient Care Center. The incumbent functions as social worker for a primary mental health cohort of veterans with both substance use disorders and other significant psychiatric disorders (including affective and psychotic disorders). The incumbent serves as a member of a multidisciplinary treatment team providing screening, assessment, individualized treatment, and case management services, as well as, family support and coordination of care with other VA and community services.</p> <p>Individual considered to be at the full performance level. Incumbent possesses the knowledge and ability to independently implement crises intervention, concrete services, and ongoing treatment modalities; including group and individual counseling, psychotherapy or educational support for individuals and families.</p> <p>The incumbent independently provides assessment and treatment of substance disorders and mental illness. Specific functions include biopsychosocial assessment, treatment planning, counseling, case management, crises management, and referral for ancillary services. Establishes and maintains effective therapeutic relationships with veterans and their families. The incumbent demonstrates expertise in standardized assessment instruments and evidence based treatment approaches (relapse prevention, cognitive behavioral interventions, as well as, risk reduction and motivational enhancement) for patients with substance and other psychiatric disorders. The incumbent utilizes assessment information and works with the patient to develop achievable goals based on identified problems with input from the interdisciplinary team. The incumbent helps the patient develop specific, time-framed, and measurable treatment objectives. In addition, the incumbent will provide advocacy and utilize resources and collaborate with community based supports to help patients maintain psychiatric stability and recovery. The incumbent provides consult on substance use assessment with level of care placement recommendations for walk-in patients, as well as, veterans identified by providers in inpatient wards and outpatient clinics.</p> <p>Independently identifies and manages high-risk patients, their psychiatric and psychosocial needs, and provides intensive case management services. Incumbent provides crisis management services and acts as liaison with involved family members. Incumbent is able to independently work with high-risk patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. Incumbent has knowledge and coordinates with ancillary VA and community services to support the multiple psychosocial needs of the veteran. Has working knowledge and experience in use of mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their</p> <p>effects/side effects, and medical terminology.</p> <p>Independently provides consultation services to other staff regarding the issues related to substance use and psychiatric disorders, and, psychosocial needs of veterans and their impact on health care and adherence to treatment plan. Ability to evaluate incumbent's own practice through professional peer review, case conferences and other organizational means.
