Working as part of the patient care team the pharmacist contributes unique expertise in medication safety, therapeutics, and education and medication preparation. The Pharmacist reports to the Pharmacy Supervisors and Directors.</p> <p>ESSENTIAL DUTIES AND RESPONSIBILITIES:</p> <p>Include but are not limited to the following:</p> <p>Provide comprehensive pharmaceutical care</p> <p>Actively promote medication safety</p> <p>Promote rational cost effective use of medications</p> <p>Provide pharmaceutical care to all types and ages of patients cared for at AMC.</p> <p>Effectively performs medication order and drug therapy review leading to improved patient outcomes</p> <p>Prepare and dispense medications</p> <p>Provide drug information</p> <p>Monitor patient drug therapy</p> <p>Collaborate with other caregivers, the patient and significant others to optimize pharmaceutical care</p> <p>Supervise and direct support personnel</p> <p>Participate in training and development of staff</p> <p>Provide professional services and when needed, leadership, to the Department, Center and Community</p> <p>Routinely work with management and others to improve pharmaceutical care and services.</p> <p>In the absence of a supervisor, can assume essential responsibilities and functions of pharmacy supervisor/manager for limited periods of time.</p> <p>Assist in the continued advancement of pharmaceutical care services.</p> <p>Willingly provide any and all appropriate functions as a recognized expert and leader.</p> <p>EDUCATION, LICENSURE, REGISTRATION AND/OR CERTIFICATION</p> <p>REQUIREMENTS:</p> <p>B.S. or Pharm.D. in pharmacy from ACPE accredited school</p> <p>Current NYS Pharmacy License.</p> <p>EXPERIENCE:</p> <p>Experience in health system pharmacy practice preferred</p> <p>Thank you for your interest in Albany Medical Center!</p> <p>Albany Medical is an equal opportunity employer.</p> <p>This role may require access to information considered sensitive to Albany Medical Center, its patients, affiliates, and partners, including but not limited to HIPAA Protected Health Information and other information regulated by Federal and New York State statutes.
