For more than fifty years, Cornerstone has been responsive to meeting the needs of the communities in which we serve with a continued emphasis on the underserved and those without access to health care regardless of race, economic status, age, sex, sexual orientation or disability.</p> <p>Description of Duties:</p> <ul> <li>Provide case management and advocacy services to an assigned caseload. </li><li>Participate in monitoring the therapeutic environment to ensure that the behavioral guidelines are followed, a therapeutic environment is maintained, and interpersonal relationships remain positive. </li><li>Provide psychoeducation and counseling appropriate to chemical dependency and relapse prevention. </li><li>Participate in assessment screenings to determine client needs/preferences; formulates and implements treatment, transition and discharge /continuing care planning collaboratively with individual clients. </li><li>Facilitate the re-socialization process of clients by serving as a role model. Provides role modeling regarding ethical and professional conduct. </li><li>Conduct facility safety checks as assigned to visually assess clients for the use of alcohol/drugs or the environment of care for unsafe conditions. </li><li>Ensure the maintenance of accurate, complete, timely and high-quality client records that comply with external regulatory standards and agency policy and procedure. </li><li>Comply with 42 CFR confidentiality and HIPAA privacy and security regulations. </li><li>Work cooperatively with other staff as a member of the interdisciplinary team. </li><li>Function as a liaison/advocate to client's families and other agencies as needed. </li><li>Provide crisis intervention, generating referrals to agency or external mental health providers or other service providers, as indicated. </li><li>Attend and participate in weekly department, monthly staff and other required meetings. </li><li>Monitoring of supervised toxicology screens as needed </li><li>Assist in peer/client chart audit reviews. </li><li>Participate in counselor of the day (COD) responsibilities. </li><li>Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family Healthcare Policy and Procedure Manual and Employee Handbook. </li><li>Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations. </li><li>Perform other duties as assigned. </li></ul> <p>Requirements</p> <ul> <li>CASAC or QHP </li><li>Bachelor's or Master's is preferred </li><li>Computer skills </li><li>Spanish speaking preferred </li></ul>
