<ul> <li> <p>JOB SUMMARY</p> </li><li> <p>Organizes and executes community programs in conjunction with the Community Health Needs Assessment that further KDMC's Mission and Vision of measurably improving the health of the community. Serves as the lead Community Services wellness educator in planning, developing, and teaching community health and wellness classes, screenings and other related activities.</p> </li><li> <p>DUTIES AND ESSENTIAL FUNCTIONS</p> </li><li> <p>Trains volunteers on educational and/or health screening equipment and completes competency validations for each volunteer. Assures that all volunteers are trained and validated for assigned duties before event when possible.</p> </li><li> <p>Overall performance rating of below indicated essential job duties:</p> </li><li> <p>Serves as leader at events, handling all aspects of event as assigned including free and Fair Market Value Screenings, including but not limited to vascular screening. Organizes and coordinates events and activities both on and off campus as scheduled.</p> </li><li> <p>Assists with Mobile Health Unit screenings and activities. Works with the drivers to ensure the units and compliance with KD policies and procedures and Joint Commission standards.</p> </li><li> <p>Teaches in schools, churches, businesses and other community based organizations regarding wellness activities including nutrition, exercise, hand hygiene, substance abuse tobacco and prevention and cessation. Assist with community health screenings.</p> </li><li> <p>Maintains teaching curriculum and hands-on educational exhibits. Makes recommendations for new purchases.</p> </li><li> <p>Works with the Community Services Specialists regarding scheduling and assignment of upcoming events. Works with the entire Community team to ensure activities are organized and coordinated.</p> </li><li> <p>Assists with the CPR Training Center and Meals on Wheels as requested.</p> </li><li> <p>Excels at speaking in front of small and large groups and is comfortable working with students and adults individually or simultaneously.</p> </li><li> <p>EDUCATION/LICENSE/ CERTIFICATIONS/OTHER REQUIREMENTS</p> </li><li> <p>Minimum requirement:</p> </li><li> <p>Associates Degree in Nursing or LPN diploma</p> </li><li> <p>5 years of nursing experience</p> </li><li> <p>Maintains current Multistate licensure as a Registered Nurse or Licensed Practical Nurse in the state of Kentucky or other NLC state as applicable.</p> </li><li> <p>Maintains a valid Driver's License</p> </li><li> <p>Preferred qualifications:</p> </li><li> <p>NA</p> </li><li> <p>WORKING ENVIRONMENT</p> </li><li> <p>Works indoors in an office setting.</p> </li><li> <p>Travel via car from location to location is required.</p> </li><li> <p>May take equipment/supplies offsite to teach at remote locations</p> </li><li> <p>The noise level is usually low to moderate.</p> </li><li> <p>PHYSICAL DEMANDS</p> </li><li> <p>Frequently required to maintain in a stationary position (standing or sitting) while preparing education or presenting to a group.</p> </li><li> <p>Frequently required to walk and move about throughout the department and Medical Center and other off-site locations..</p> </li><li> <p>Frequently required to operate a computer and other equipment, such as A/V equipment.</p> </li><li> <p>Frequently required to use fine motor skills to handle and operate instruments/devices</p> </li><li> <p>Constantly required to lift and/or move items up to 10 pounds.</p> </li><li> <p>Frequently required to lift and/or move items up to 25 pounds.</p> </li><li> <p>Occasionally required to lift and/or move items up to 50 pounds.</p> </li><li> <p>Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception and the ability to adjust focus</p> </li></ul>
