<p>Position Summary Under the direct supervision of the Director of Substance/Opioid Use Team, this Substance Use Nurse Navigator, will participate in the facilitation of medicated assisted treatment (MAT) in addition to the promotion and restoration of patient's health through collaboration with physicians and multidisciplinary team members with the emergency department, hospital, and local community. The nurse will be responsible for providing recommendations for induction of MAT and providing support to the patient for continued treatment at discharge. The navigator will work in collaboration with ED staff, pharmacy champion, hospital navigator and local community partners to provide best practices for patients with substance use disorders. Responsibilities • Establishes processes and protocols for appropriate referrals to treatment for medicated assisted treatment initiation • Follows up with patients on engagement in treatment and provides additional resources to the patient and/or family. • Educates patients, families, providers, and staff on opioid/substance use disorder and treatment options. • Secures all necessary releases from clients/patients to report all data collected on contacts, referral services, and follow-up. • Develops programs to support patients and families dealing with opioid addiction and substance use disorders. • Utilizes local, state, and federal programs to facilitate referrals and care coordination. • Follows up on inpatient referrals in the community and collaborates with the OUD/SUD team for patient recovery. • Initiates Baker Acts/Marchman Acts as appropriate. • Supports ED staff in triage and treatment of all patients with identified OUD/SUD • Acts as a consultant and educator to multidisciplinary teams on opioid/substance use disorders. • Demonstrates excellent interpersonal and written communication skills. • Works autonomously and self-directed while collaborating with other team members. • Exhibits strong negotiation and advocacy skills with patients, families, team members, health care team, and community resources advocating for MAT for patients with Opioid Use Disorders. • Demonstrates knowledge and skill in data preparation, analysis, and presentation to maintain effective, efficient, and compliant work environments.
