Supports patients and families by providing care management during transitions of care. Assists Primary Care Physicians in assessing need and utilization of home health services.</p> <p>Job Requirements</p> <p>In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:</p> <p>Experience Requirements:</p> <ul> <li>Three to five years care management experience or a combination of relevant experience </li><li>Experience in health care or community setting working with geriatrics, i.e. case management, home health care, discharge planning or department of social services </li><li>Experience with Care Transitions coaching model, Motivational Interviewing and managing psych/social issues with patients and families </li><li>Knowledge of case management, community resources/agencies, NCQA, QA, Medicare/Medicaid guidelines, program and workflow development, process improvement and implementation. </li></ul> <p>Education Requirements:</p> <ul> <li>MSW or Master's Degree in related fields such as counseling, gerontology etc. </li></ul> <p>License/Certification:</p> <ul> <li>LSW or LCSW Preferred </li><li>Certification in Care Management preferred </li><li>Current BLS certification required from American Heart Association required </li></ul> <p>Physical Requirements:</p> <p>Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally</p> <p>Where You'll Work</p> <p>CommonSpirit Health Mountain Region provides home care, end-of-life-care and operates two senior living communities.
