The salary range for this position is $62,186.00 - $72,500.00. There are two fixed furlough days for all limited (temporary positions with an end date) and unlimited (permanent) employees on August 29th, the Friday before Labor Day, and November 28th, the day after Thanksgiving. If you receive an offer for this position, your annual pay will determine whether you are required to take additional furlough days. In this position, you can expect to work mostly in the field, and within the office when needed. The office for this position is located at 201 W. Colfax Ave., Denver, CO 80202.</p> <p>What You'll Do</p> <p>Denver's Department of Public Health and Environment (DDPHE) is dedicated to advancing Denver's environmental and public health goals. DDPHE works collaboratively with city, state and community partners to conduct education, community engagement, and enforcement to promote healthy people, healthy pets, and a sustainable environment. DDPHE oversees a number of services related to public health and the environment and is comprised of seven divisions: Division of Shared Services and Business Operations; Community & Behavioral Health; Public Health Investigations; Denver Animal Protection; the Office of Medical Examiner; Emergency Management and Medical Operations; and Environmental Quality. For more information on DDPHE's programs and services, visit the DDPHE website at: Denver Department of Public Health and Environment.</p> <p>We foster an inclusive culture by respecting the values, customs, and authenticity of everyone and by implementing equitable public health strategies so Denver communities can thrive.</p> <p>As the Behavioral Health Care Coordinator, you will work with individuals identified by City of Denver Partners, through street engagement, placement at All-In Mile High Sites, and various behavioral health programs, to identify a person's behavioral health and wellness needs and support immediate access to care.? The Behavioral Health Care Coordinator will be responsible for determining service availability, completing the referral process, securing transportation, and complete a face-to-face handoff to the service provider. This position will also be a reliable contact for service providers if there are concerns with an individual in treatment in their initial admission period.</p> <p>R2R supports Mayor Johnston's 2025 "All In Mile High Goal" to bring 2,000 people off the streets and into temporary housing, and another 2,000 into permanent housing. R2R continues to be a coordinated effort towards an intervention, treatment and rehabilitations pipeline.</p> <p>Specifically, as the Behavioral Health Care Coordinator with Roads to Recovery, you will:</p> <ul> <li> <p>Support individuals with determining appropriate level of care and corresponding supportive services, with needs that include transiency, behavioral and mental health disorders, substance use disorders, medical health, job readiness and education, and criminal justice involvement.</p> </li><li> <p>Work as a team amongst Denver City agencies to include The Office of Social Equity and Innovation, The Department of Safety, Department of Housing Stability, Department of Transportation & Infrastructure, Department of Development & Economic Opportunity, and other programs managed by the Department of Public Health and Environment.</p> </li><li> <p>Maintain positive and co-working relationships with community partners and health providers.</p> </li><li> <p>Meticulously track client data, client engagement, and client outcomes.</p> </li><li> <p>Transportation of clients to placements, resources, or services.</p> </li><li> <p>Serve as a point person for assessments, referrals and follow up with the provider.</p> </li><li> <p>Communicate with contract managers on provider outcomes.</p> </li><li> <p>Participate in team meetings, individual and group consultation, and community-based learning opportunities.</p> </li><li> <p>Along with all department staff, this position will actively participate on an emergency preparedness and response team to support DDPHE's responsibilities outlined in the City's Emergency Operations Plan. This may require being 'on-call' from time to time to help respond to emergencies.
