This position is based in Charlestown and is a field based position supporting patients in the community / Eastern MA .Job Description:Essential Duties & Responsibilities including but not limited to:Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the homeNavigating patients and families across the continuum of the Medically Home phases of careMonitoring and responding to biometric data per Medically Home protocolsConducting home and audio/visual patient care visits as directed by the care team and/or protocolEnsuring all interventions in the patient's care plan are implemented efficiently with a patient centered approachLeveraging the resources and expertise of the Medically Home team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundationAdhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based carePromoting professional practice and a culture of safety; willingness to engage in process improvement efforts Clinical Roles and Responsibilities: Providing protocol driven assessment, care coordination and interventions as appropriate informed by Medically Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative careExhibiting sound clinical decision making and critical thinking skillsMaintain and practice in accordance with nursing guidelines per state and hospital regulationsFacilitating admissions to Medically Home utilizing MH guidelinesServing as a role model for professional nursing practiceUtilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problemsSharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertiseSupporting research efforts to advance knowledge and promote evidence-based practicePreforming other duties and responsibilities as assigned by the Senior Director of NursingQuality and Safety Responsibilities:Ensure compliance with reporting requirements for Sentinel Events and Serious IncidentsParticipate in quality and safety investigations and root cause analysesTake appropriate action to ensure patient safety as a result of any investigation; andServe as a resource in quality improvement processes and techniqueOrganizational Requirements:Maintains strict adherence to the Lahey Clinic Confidentiality Policy. Incorporates Lahey Clinic Guiding Principles, Mission Statement and Goals into daily activities. Complies with all Lahey Clinic Policies. Complies with behavioral expectations of the department and Lahey Clinic. Maintains courteous and effective interactions with colleagues and patients. Demonstrates an understanding of the job description, performance expectations, and competency assessment. Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards. Participates in departmental and/or interdepartmental quality improvement activities. Participates in and successfully completes Mandatory Education. § Performs all other duties as needed or directed to meet the needs of the department.Minimum Qualifications:Education: The minimum post-secondary education requirement to become a Registered Nurse is a Bachelor of Science degreeLicensure, Certification, Registration:A RN current licenseSkills, Knowledge & Abilities:Clinical knowledgeAble to promote patient safety and a positive patient experienceFlexible and comfortable with a changing and dynamic work environment and schedule across all shifts and daysTeam player who also works well independentlyParticipates in ongoing learning and continuing education to care for a variety of patient populationsCollaborate, advocate and mentor others with a professional, non-condescending mannerModel collaborative, respectful team relationships with colleagues and customers; lead by exampleStrong interpersonal, inter-professional communication and collaboration capabilitiesAbility to analyze, interpret and problem solve quickly with well-articulated action planningStrong verbal/written communication and conflict resolution skillsStrong empathy to deliver compassionate, patient centered care Experience: Three (3) years RN experience Pay Range: $76,960.00 - $199,139.20The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
