<p>NURSE II</p> <p>GENERAL SUMMARY</p> <p>The purpose of this job is to provide medical and nursing services to Juvenile Home residents which may include but is not limited to immunizations, infectious disease testing, and administering medications. Performs assessments, completes referrals, and maintains documentation. Performs care coordination and health education.</p> <p>ESSENTIAL DUTIES & RESPONSIBILITIES</p> <ul> <li>This position serves as the primary decision maker in assessing and determining care interventions. </li><li>Performs case management duties such as ongoing patient assessment of Juvenile Home residents, coordinating services with residents and vendors. Administers immunizations. </li><li>Maintains documentation of assessments, collaborates with staff and family to promote resident health needs, and facilitates continuum of care in preparation for the youth's return home. </li><li>Dispenses medication, assesses medical conditions, and coordinates emergency medical care for residents and staff as needed. </li><li>Coordinates referrals for outpatient appointments, arranges transportation with probation officers, and documents completion of the appointments. </li><li>Evaluates the preventative healthcare needs of Juvenile Home residents and performs a baseline physical exam within 7 days of admission. </li><li>Maintains medical inventory and supplies; manages and secures resident medications; plans medication distribution schedule, maintains medication documentation, and monitors patients' responses to medications. Communicates concerns and changes needed to prescribing physicians. </li><li>Conducts staff training and education, including, but not limited to, training in CPR/AED/First Aid, Bloodborne Pathogens. Performs annual Tuberculosis testing on all staff and maintains testing and training records. Updates Juvenile Home infection control policies/education as recommended by the local health department and the CDC. </li><li>Coordinates and provides health/hygiene educational opportunities for Juvenile Home residents to promote positive health behaviors. </li><li>Performs related work as required. </li></ul> <p>MINIMUM QUALIFICATIONS</p> <p>Required Education and Experience</p> <ul> <li>Bachelor's degree from an accredited college or university in Nursing </li><li>3 to 5 years of related work experience in nursing, caring for adolescents, public health nursing, or in an acute care setting </li></ul> <p>Required Licenses or Certifications</p> <ul> <li>Registered Nurse in the state of Michigan </li></ul> <p>Veterans are encouraged to apply.
