<p>Overview</p> <p>Provides assessment, support, information and referral, advocacy services, and other resource assistance to patients and their caregivers, directly or through consultation, within a multi-disciplinary outpatient team.</p> <p>SIGN ON BONUS ELIGIBLE: $7,500</p> <ul> <li>CONDITIONS APPLY </li></ul> <p>Responsibilities</p> <p>Provides support to pediatric patients and families, in an outpatient clinic setting as part of a multidisciplinary team, to help address psychosocial needs within the context of the patient's medical care. Completes psychosocial assessments of pediatric patients and families to assess current supports, strengths, areas of need, and barriers to accessing care. Coordinates and connects patients and their caregivers with community resources to address/meet identified needs. Serves as a link between the medical team and the patient's community/natural supports. Assists caregivers in adapting to difficult and complicated dispositions involving complex care needs, multiple agencies/providers and linkage to resources in the community. Provides consultation, assessment, referral and coordination in cases of suspected child abuse or neglect. Organizes support groups, provides psychoeducational materials and opportunities for patients and their caregivers, based on their individual medical needs. Documents objective, concise and pertinent clinical information in the medical record. Perform other duties as assigned.</p> <p>Qualifications</p> <ul> <li>Master's Degree in Social Work (MSW) required. </li><li>Social work experience in an acute, home care, or ambulatory setting preferred. </li><li>Strong organizational skills, verbal and written communications skills. </li><li>Sound decision making and judgment. </li><li>Time management skills. </li><li>Negotiation skills. </li><li>Proven ability to deal with conflict resolution. </li><li>Comfortable with basic computer skills. </li></ul> <p>Required Licensure/Certifications</p> <ul> <li>None </li></ul>
