Provides Palliative care clinical consultation.</p> <p>What You Will Do:</p> <ul> <li>Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation </li><li>Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy </li><li>Provides statistical/budgetary data to appropriate Palliative care program </li><li>Works collaboratively with all departments to ensure execution of quality delivery of services </li></ul> <p>Minimum Qualifications:</p> <ul> <li>Graduate of an approved nursing education program. </li><li>Current Registered Nurse licensure in the state of practice. </li><li>CPR certification </li><li>Hospice/Palliative Care certification preferred. </li><li>Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills, </li><li>Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills </li><li>Must be able to work independently and on a team </li><li>Ability to perform diversified duties with time limitations </li><li>Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care </li></ul> <p>Position Highlights and Benefits:</p> <ul> <li>Medical, dental and vision insurance - day one benefits </li><li>Short and long-term disability </li><li>403b with matching contribution </li><li>Generous paid time off PLUS 7 paid holidays </li><li>Comprehensive orientation </li><li>Tuition reimbursement up to $5,250 a year </li><li>Pay range: $36.71-$58.71 </li></ul> <p>Ministry/Facility Information:</p> <p>Loyola Medicine Home Care and Hospice is a faith-based, non-profit organization dedicated to holistic, patient-centered care.
