<p>PHMC is proud to be a leader in public health. PHMC requires all employees to be fully vaccinated for COVID-19 before the first day of employment. We will offer the vaccination at no cost via our Health Centers.</p> <p>JOB OVERVIEW:</p> <p>The Community Engagement Manager establishes and maintains mutually beneficial relationships with community stakeholders to increase resource support for community members surrounding PHMC programs and subsidiaries. This role collaborates closely with PHMC outreach staff across the enterprise highlighting ongoing community work and resources. The position supports the coordination of organizational outreach and awareness initiatives by planning and implementing strategies and activities to expand the reach of PHMC programming. The position focuses on enhancing community engagement strategies, providing essential resource connections, and promoting collaboration between residents, PHMC, and community partners. The Community Engagement Manager provides key oversight and leadership to the Community Engagement Team and reports to the Director of Community Engagement.</p> <p>Responsibilities:</p> <p>Leadership</p> <ul> <li>Supervise the daily responsibilities Care Concierges and youth navigators. </li><li>Lead the planning and execution of PHMC Day. Including but not limited to: Designate planning teams based on key focus areas (i.e. Policy/Advocacy, activities, logistics, etc.) and delegate appropriate tasks to team leads/members. Schedule regular meetings to ensure that teams remain on task and that desired outcomes are met. </li><li>Lead fundraising efforts related to NNCC Community Action Day including developing sponsorship strategy and partnerships between NNCC and external community partners with measurable outcomes to evaluate success. </li><li>Lead development activities related to community engagement at PHMC. Working closely with Director of Community Engagement, this role will help determine fundraising needs for specific initiatives such as PHMC Day. Develop a fundraising plan and engage sponsors by building and maintaining relationships and organizing fundraising events. </li></ul> <p>Support the Expansion of Community Engagement Function</p> <ul> <li>Support the enhancement of NNCC outreach and resource navigation efforts by sharing best practices and collaborating on external client-facing communication efforts. </li></ul> <p>Partnership Development and Stakeholder Engagement</p> <ul> <li>Assist in the expansion of community engagement across the PHMC enterprise in collaboration with the Director of Community Engagement </li><li>Assist in the development of partnerships and MOUs with community-based social services agencies and other organizations that will benefit PHMC program participants and their families. </li><li>Build trusting relationships with clients, community partners, and other external stakeholders by practicing trauma-informed principles and remaining consistent, reliable, and responsive. </li><li>Coordinate community engagement initiatives with internal and external partners. </li></ul> <p>Client Services and Resource Navigation</p> <ul> <li>Lead the implementation of record audits and quality improvement strategies and contribute to PHMC strategic planning initiatives to sustain and grow resource navigation. </li><li>Manage resource navigation services with clients referred by PHMC programs and subsidiaries. </li><li>Remain current on existing and new resources that could benefit the families of PHMC programs. Maintain the resource database. </li><li>Provide oversight and maintenance of documented encounters with clients, referring staff, and other service providers in the applicable database on time and communicate resource navigation needs related to data collected to Community Engagement leadership. </li><li>Maintain and review database reports regularly to meet funder requirements and lead continuous quality improvement initiatives. </li></ul> <p>Program Development and Community Feedback</p> <ul> <li>Develop and execute targeted events based on formal and informal client feedback and programmatic data related to resource needs to increase participant and community involvement and create a welcoming environment for participants aligned with PHMC's mission and values. </li><li>Participate in the development and execution of client feedback initiatives related to the client resource navigation experience. </li><li>Work collaboratively on special initiatives such as the Community Advisory Board, Auxiliary Board and community research advisory board. Participate in recruitment, development, and planning meetings with internal staff. Attend and facilitate monthly PHMC Community Outreach Committee Meetings </li></ul> <p>Organizational Support and Development</p> <ul> <li>Offer information sessions about resource-sharing initiatives. Function as expert liaison with staff and community groups. </li><li>Contribute to a positive work environment that engages community-based organizations and other entities with PHMC programs and related initiatives. </li><li>Assume other responsibilities as necessary. </li></ul> <p>Skills:</p> <ul> <li>Detail-oriented, able to focus on the task at hand, catch errors, and fix them efficiently and professionally. </li><li>Ability to work independently, be proactive, identify challenges and create solutions </li><li>Ability to work as apart of a team, collaboratively and open to feedback. </li><li>Dedicated to working with professionals, families, community members, and Philadelphia-based organizations. </li><li>Committed to continually building on advanced communication skills and resource navigation process to engage clients, families, and organizations. </li><li>Commits to culturally respective orientation. Respect the diversity of the communities and clients served. </li></ul> <p>WORK CONDITIONS:</p> <ul> <li>Flexible schedule including some early evenings and weekends for special events (on-site and off-site). </li><li>Light to moderate lifting and transport of program/client materials </li><li>Light to moderate exercise including walking throughout the day, stair climbing, use of transportation services. </li><li>Use of public transit or private transportation as needed local or regional travel as required by funders or senior staff. </li></ul> <p>QUALIFICATIONS:</p> <ul> <li>Work clearances that include FBI, Commonwealth of Pennsylvania's Child Clearance, Criminal Check, and others required by Public Health Management Corporation </li><li>If possesses an automobile and uses it for work-related activities, has valid and current driver's license and automobile insurance. </li><li>Able to work on-site per assignments/commitments. </li><li>Minimum of 3 years of related professional experience, including public health, nursing programs, maternalchild health, integrated health systems, or related fields. </li><li>Knowledge of Philadelphia neighborhoods and the city at large. </li></ul> <p>EDUCATION REQUIREMENT:</p> <ul> <li>Collegiate social work/public health studies. </li><li>Associate's degree in a relevant field. </li><li>Enrolled in or completed baccalaureate program preferred. </li><li>Continuing education and certificates related to social work, communication, public health, and others applicable to this position. </li></ul> <p>SALARY:</p> <p>Commensurate with education and experience.</p>
