<p>Responsibilities:</p> <ul> <li>Designs and facilitates the Community Memory Care program, promoting active participation from residents, family members, and team members </li><li>Oversees the budget and implementation of a cost-effective Memory Care program, including managing an inventory list of equipment and supplies and maximizing resources available </li><li>Creates and sustains a welcoming and safe environment which promotes resident wellness and independence </li><li>Participates in the resident move-in process through initial wellness assessments and the development of individualized care plans </li><li>Trains, schedules, and leads the Memory Care clinical team in providing exceptional care experiences, providing hands-on support, education and training opportunities </li><li>Delivers health promotion services and dementia care education services for residents and families </li><li>Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience </li><li>Contributes toward quality assurance and performance improvement initiatives </li><li>Coordinates the Community's infection control program while maintaining state regulatory compliance </li></ul>
