This grant-funded position will be responsible for achieving the PASOs Health Connections program goals assigned for the Tri-County region of South Carolina.</p> <p>Job Related Minimum Required Education and Experience</p> <p>Requires a bachelor's degree in a job related field and 1 or more years of job related experience, which may be substituted by an equivalent combination of job related certification, training, education, and/or experience.</p> <p>Required Certification, Licensure/Other Credentials Preferred Qualifications</p> <p>Degree in Health education, health promotion, or a closely related field such as psychology, social work biology, sociology, or education.</p> <p>Knowledge/Skills/Abilities</p> <ul> <li>Ability to use intermediate functions and features of the MS Office Suite, including Outlook, Word, Excel, and PowerPoint. </li><li>Ability to navigate the internet, including usage of and the ability to successfully and accurately utilize web-based programs and applications. </li><li>Ability to learn and effectively use electronic medical record software. </li></ul> <p>Job Duties</p> <p>Job Duty</p> <p>Serve as an information source for Latino families referring them to local resources and services, and answering their questions when possible. Remain abreast of federal, state, and local community resources to provide interventions that address access to care and social determinants of health for participants and assist them in enrolling in local programs for which they qualify. Assist individuals and families with assessing program affordability, eligibility requirements, education about programs, and enrollment in various benefit programs including but not limited to Medicaid, WIC, and SNAP.
