<p>Mountain Community Health Partnership (MCHP) is seeking a full time Care Coordinator to join our Care Management team. The Care Coordinator will work collaboratively to support patient care plans and needs, serving as an advocate for patients. This role involves coordinating between healthcare services and recognizing needs of patients, including social and cultural dynamics. The Care Coordinator will support MCHP goals and objectives in meeting improvement targets, standardizing the plan of care, and supporting team development.</p> <p>Key Responsibilities:</p> <ul> <li>Engage with patients to identify and address barriers that impede health outcomes. </li><li>Implement and support Care Management interventions per the patient's care plan or assessed community needs. </li><li>Process referrals from multidisciplinary team members accurately and timely. </li><li>Ensure patients receive screenings, care and follow-up. </li><li>Schedule and verify medical appointments for patients as needed. </li><li>Provide education to patients/family within scope of practice. </li><li>Adhere to MCHP privacy and security policies and HIPAA regulations. </li></ul> <p>Qualifications:</p> <ul> <li>Education: High School graduate or GED; completion of certificate program in healthcare area of focus. A 2-year degree in human services or nursing field is preferred. </li><li>Experience: Minimum of three (3) years' experience in an ambulatory/care management or acute care setting, home health or public health. </li><li>Proficiency in Microsoft Office, excellent verbal and written communication skills, organizational, problem-solving, and critical thinking skills.
