<p>This position has a sign on bonus of $7,500!</p> <p>SUMMARY</p> <p>The MDS Coordinator, RN supports the applicable clinical software used in assessments and care planning, with a</p> <p>goal of facilitating quality assurance.</p> <p>PRIMARY RESPONSIBILITIES</p> <p>1) Oversee and administer the Resident Assessment process according to applicable policies and</p> <p>regulations.</p> <p>a. Schedule the ARDs (Assessment Reference Date) for payment assessments in a manner that</p> <p>accurately captures the RUG (Resource Utilization Group) category. Provide a schedule of ARDs and</p> <p>assessment types to the IDT (interdisciplinary team) to facilitate timely completion of MDS sections</p> <p>and CAAs (Care Area Assessments) by each discipline.</p> <p>b. Schedule all OBRA (Omnibus Budget Reconciliation Act) ARDs (assessment reference dates.</p> <p>Provide schedule to IDT. Schedule admission, quarterly and annual assessments. Provide</p> <p>addendums to the schedule to the IDT as needed as related to COTs, discharge assessments and</p> <p>SCSA (Significant Change in Status Assessments).</p> <p>c. MDS sections as assigned facilitate with IDT members the completion of assessments.</p> <p>d. Ensure the transmission of assessments per regulation.</p> <p>2) Facilitate the completion and updating of resident Care Plans within MDS and the IDT to reflect the</p> <p>resident's most current needs.</p> <p>a. Provide ongoing leadership for data accuracy, data submission and computer applications of Resident</p> <p>Assessment, communicating regularly with the IDT.</p> <p>3) Coordinate skilled coverage for residents with managed care.</p> <p>a. Obtain authorization when indicated and provide updates as requested.</p> <p>4) Review Medicare Part A coverage and necessary documentation for eligible residents.</p> <p>5) Obtain signed physician certification forms as indicated.</p> <p>6) Participate in quality improvement initiatives as directed.</p> <p>7) Provide necessary documentation of all activities to meet regulations.</p> <p>a. Maintain validation reports in a transmittal notebook organized by month.</p> <p>8) Support various other Nursing responsibilities as needed.</p> <ul> <li>Denotes Essential Functions of the position. </li></ul> <p>JOB SPECIFICATIONS</p> <p>Education, Experience and Credentials</p> <p>Licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.</p> <p>One or more years' nursing experience in a long-term care setting required.</p> <p>One year of MDS and care plan development experience preferred.</p> <p>CPR Certification preferred.</p> <p>Knowledge, Skills and Abilities</p> <p>Flexible and productive in an environment with regular interruptions.</p> <p>Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of</p> <p>individuals both internal and external to the organization.</p> <p>Possess effective problem solving skills.</p> <p>Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must</p> <p>be skilled in database management and record keeping.</p> <p>PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB</p> <p>Incumbent must be able to:</p> <p>Perform light work, lifting up to 20 pounds, with frequent lifting/carrying of objects weighing up to 10 pounds.</p> <p>Push and move wheelchairs with residents of all weights and sizes for short and long distances.</p> <p>Stand, walk, twist, turn, bend, stoop, kneel, and reach frequently in the performance of resident assistance</p> <p>and nursing duties.</p> <p>Effectively communicate in the English language, including following oral and written instructions, in order to</p> <p>communicate with people inside and outside the organization.</p> <p>Work at a desk for extended periods and perform hours of work at a computer.</p> <p>Apply manual dexterity and computer keyboarding and office equipment use.</p> <p>Perform repetitive movements of the upper extremities.</p> <p>Perform frequent use of hands for firm gasping and fine manipulating.</p> <p>TOOLS AND EQUIPMENT USED</p> <p>Use standard nursing equipment such as thermometer, stethoscope, blood pressure cuff, wheelchair, patient</p> <p>lifts, and related medical equipment.</p> <p>Use telephone, computer and standard office equipment.</p> <p>Sholom Community Alliance</p> <p>WORK ENVIRONMENT</p> <p>The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to</p> <p>respond to the needs of residents, in responding to emergencies.
