<p>Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team.</p> <p>This RN will be working with the Heart Failure and Advanced Heart Failure team in addition to working with LVAD and heart transplant patients</p> <ul> <li>Development and implementation of IPD critical Pathways </li><li>IPD discharge planning </li><li>Facilitating and managing the use of home care </li><li>Triage of OPD and IPD medical issues with referral to appropriate health care professionals </li><li>Communications with insurance companies and referring physicians </li><li>Quality assurance projects </li><li>Promoting health maintenance education and follow-up </li><li>Donor & Recipient Family and patient management </li></ul> <p>Location: Detroit</p> <p>Hours: 630AM-5PM, 4- 10-hour shifts.
