If transporting students, must complete and maintain all qualifications for Ohio Van Drivers License (ODE rules).</p> </li><li> <p>Assist the worker in learning all job tasks.</p> </li><li> <p>Break down job tasks and offer supports/modifications with the assistance of the Transition to Work Coordinator and classroom teachers.</p> </li><li> <p>Learn and demonstrate specific coaching skills as well as the ability to fade support to foster independence.</p> </li><li> <p>Serve as a liaison between the supervisors on the job, the student and the school team.</p> </li><li> <p>Identify and help establish natural supports on the job site.</p> </li><li> <p>Supervise student on the job at all times and follow written, approved fade out plan.</p> </li><li> <p>Communicate worker's progress to the Transition to Work Coordinator, Teacher and employer (if appropriate).</p> </li><li> <p>Keep data sheets on worker progress or issues.</p> </li><li> <p>Discuss modifications attempted and results of these modifications.</p> </li><li> <p>Discuss anticipated plan and direction of training with the Transition to Work Coordinator.</p> </li><li> <p>Provide suggestions and input regarding worker training to the Transition to Work Coordinator or Teacher.</p> </li><li> <p>Keep accurate records of service delivery for purposes of billing student's school district, mileage reimbursement forms, and time sheets; turn in these documents in a timely manner.</p> </li><li> <p>Participate in meetings regarding the student as requested.</p> </li><li> <p>Maintains student confidentiality, providing only information to authorized persons.</p> </li></ul> <p>EQUAL EMPLOYMENT OPPORTUNITY AND ADA STATEMENT:</p> <p>The Hamilton County Educational Service Center is an Equal Opportunity Employer and endeavors to provide the highest quality service through employing professionals selected for their competence and expertise.
