<p>PRIMARY ACCOUNTABILITIES:</p> <ol> <li> <p>Provides confidential assessment to determine the HIV prevention needs to the patient.</p> </li><li> <p>Take the lead with those testing negative to ensure the patient is educated about safe behaviors and PrEP, receives necessary lab work to start PrEP, and is assisted with the costs of the medication so as not to be a barrier to adherence.</p> </li><li> <p>Provides prevention services ensuring access, follow-up, and adherence to PrEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance.</p> </li><li> <p>Support the delivery of integrated primary and HIV care services, perform linkage to cares services, and improve health equity by engaging multiple systems of care to provide an integrated and coordinated approach to improve overall medical and behavioral outcomes for people living with HIV and highly vulnerable HIV negative persons.</p> </li></ol> <p>Operational Excellence</p> <ul> <li>Ensure all tasks provided and associated with patient care, patient administrative processes, and related duties complies with all regulatory and accreditation standards, as well as clinic policies and procedures. </li></ul> <p>Relationships</p> <ul> <li>Develop favorable relationships with all patients. Instruct patients and their family members education and /or information related to connection with prevention and treatment of HIV and or substances/abuse. </li><li>Establish favorable working relationships with all staff members associated with center operations, including pharmacist. </li></ul> <p>Stewardship and Professionalism</p> <ul> <li>Uphold and consistently represent the values and mission of the organization always. Represent the organization in a highly professional manner always. </li><li>Ensure compliance and attention to all corporate policies and procedures. </li></ul> <p>PRIMARY TASKS & DUTIES:</p> <ul> <li>Establish patient eligibility and patient readiness through the use of a screening tool, arrange for laboratory collection, and schedule an appointment for the initial Provider visit. </li><li>Assist all patients through the healthcare system by acting as a patient advocate and navigator. </li><li>Data collection, health outcomes reporting. </li><li>Offer partner services to individuals who test positive for HIV. </li><li>Perform risk assessments with individuals who test positive and negative for HIV to reinforce realistic perception of risk. </li><li>Support patient self-management of disease and behavior modification interventions. </li><li>Coordinate continuity of patient care with external healthcare organizations and facilities. </li><li>Promote clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans. </li><li>Follows through with post-visit duties. </li><li>Assists with office duties as assigned. </li><li>Other duties as assigned. </li></ul> <p>Great work atmosphere and excellent pay and benefits include 4 weeks paid time off, 10 paid holidays, BCBS Health Insurance, GAP Health Insurance, Vision, Dental, Life, STD, LTD, Critical Illness, Retirement Plan.
