The Clinical Coordinator will be assigned Care Managers and will offer orientation, education, training, mentoring, and support. The Clinical Coordinator will maintain an active partial caseload performing all duties related to direct supports coordination of assigned consumers.</p> <ul> <li>Assist Program Manager with daily operations of department </li><li>Provide supervision, training and support to Care Managers </li><li>Convey relevant information to Program Manager concerning departmental operations, staff concerns, budget issues, and personnel matters. </li><li>Review all annual and critical revision ISP's prior to submission for assigned Care Managers. </li><li>Track unit productivity, documentation quality, monitoring completion and ISP status for all assigned Care Managers </li><li>Assist with oversight of departmental procedures to assure compliance with Bucks and Montgomery County AE office, State, Federal, and St. Luke's policies and procedures. </li><li>Assist with development and analysis of service needs of consumers </li><li>Oversee orientation and training of assigned Care Managers </li><li>Maintain regular scheduled supervision with assigned Care Managers </li><li>Provide as needed, supervision for Care Managers when required, including onsite assistance and support for meetings and monitoring </li><li>Assist Program Manager with issues related to ID services, programs and needs </li><li>Assist Program Manager with assignment of cases and tasks to staff </li><li>Assist staff with skill development, resource utilization, and be available for onsite assistance </li></ul> <p>EDUCATION/EXPERIENCE:</p> <ul> <li>Bachelor's Degree in related field plus three years of related work experience within the Intellectual Disabilities system or related field. </li><li>Two years of experience as a Care Manager (Supports Coordinator) in the field of intellectual disabilities. </li></ul> <p>PHYSICIAL AND SENSORY REQUIREMENTS:</p> <p>Sit up to 2 hours per day; 1 hour at a time. Stand for up to 8 hours per day, 4 hours at a time. Walk 6 hours per day; 10 minutes at a time. Consistently lift, carry and push objects up to 10 lbs. Frequently lift, carry and push objects up to 75 lbs. Transport patients weighing up to 250 lbs. via wheelchair, bed and/or stretcher with assistance when appropriate. Consistently pull up to 10 lbs. Frequently pull up to 250 lbs. with assistance when appropriate. Frequently stoop and bend, crouch, kneel and climb. Frequently reach above shoulder level. Occasional crawling. Must be able to perceive attributes of an object through touch. Frequently finger and handle objects. Occasionally firmly grasp, twist, and turn objects with hands and fingers. Must be able to hear as it relates to normal conversation, and high and low frequencies. Must be able to see as it relates to general, near, far color, and peripheral vision, depth perception, and visual monotony (e.g. computer screen).</p> <p>Please complete your application using your full legal name and current home address.
