<p>The Therapist, Correctional Facility will provide therapeutic clinical interventions to a special population of adults at the county jail. In addition, this position provides support through appropriate assessments and quality of services within the project. This position reports to the Team Leader, Correctional Facility.</p> <p>ESSENTIAL DUTIES & RESPONSIBILITIES</p> <ul> <li>Provides direct service to adult patients assigned to the team through individual counseling, coaching, education, case consultation, and crisis intervention. </li><li>Provides recovery-oriented therapeutic interventions to support the patient with the management of symptoms and daily functioning while patients await state hospital placement for fitness restoration. </li><li>Coordinates treatment with the Team Lead, prescriber and designated Department of Mental Health (DMH), county jail corrections and contracted health care staff and pharmacy. </li><li>Attend weekly care coordination progress meetings with DMH and county jail staff. </li><li>Completes and maintains all required agency and court documentation. Maintains the quality, validity and confidentiality of individual case records. Assures that all assigned documentation is completed accurately, on time and is organized in a systematic manner. </li><li>Provides crisis intervention services as needed for patients. </li><li>Maintain the quality and validity of reports and individual case records.
