<p>Requisition #:211670</p> <p>Location:Plymouth, MA 02360</p> <p>Category:PT</p> <p>Salary:$65.00 per hour</p> <p>Position Details</p> <p>Our Visiting Physical Therapist for our VNA Certified division is responsible and accountable for evaluation, coordination, and administration of quality physical therapy services.</p> <p>At Aveanna, our services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients' homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs.</p> <p>Pay</p> <p>$65 per hour/visit based on the pay point model</p> <p>Why Join Us?</p> <ul> <li>Flexible scheduling- full-time & part-time. If you meet the requirement under the twelve-month Measurement Period, you will also be eligible for your choice of major medical plans, dental and vision coverage. </li><li>Benefit eligibility can vary and is dependent upon employment status and employment location. </li></ul> <p>What You'll Do</p> <ul> <li>Delivery of physical therapy services to comply with prescribed physician orders. </li><li>Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws. </li><li>Provide a written evaluation defining physical therapy needs, goals, progress and expected outcomes in conjunction with referring physician's Plan of Treatment. </li><li>Participates in coordination of services (including discharge planning) with health care team, physicians, parent/guardians, nurses, etc. </li><li>Maintain effective communication between staff, healthcare team members and family. </li><li>Serve as a resource consultant for clinical staff. </li><li>Supervises other personnel (PTA, HHA) as applicable. </li><li>Completes OASIS assessments where allowed by state professional practice and regulation. </li></ul> <p>Qualifications</p> <ul> <li>A certification in Physical Therapy in the state of application </li><li>Current and unrestricted license in the state of practice </li><li>Current CPR certification (with hands-on component) or able to obtain one </li><li>Current TB test or able to obtain one </li><li>Must have a drivers license and reliable transportation </li></ul> <p>Aveanna Healthcare is an Equal Opportunity Employer and encourages applicants from diverse backgrounds to apply.</p> <p>As an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate.</p>
