<p>SPECIFIC DUTIES AND RESPONSIBILITIES</p> <p>ESSENTIAL JOB FUNCTIONS</p> <ul> <li>Facilitates pre-enrollment tasks to prepare for a client's enrollment. </li><li>Facilitates the admission intake process. </li><li>Case Managers develop the Treatment team/Medication Meeting schedule </li><li>Facilitates multidisciplinary team meetings to monitor client progress and updates the Plan of Care. </li><li>Meets and adheres to all timelines as required for funding (i.e. Medicaid EPSDT/Waiver, Private Insurance and Single Case Agreements) </li><li>Coordinates and facilitates the new IEP (students) or PCP (adults) and revisions to those documents. </li><li>Ensures documentation for each Client's Record is accurate and properly maintained. </li><li>Managing the Transition and Discharge Process </li><li>Facilitates discharge from Grafton. </li><li>Facilitates transfer to another Grafton program or level of service provided by Grafton. </li><li>Prepares for and attends FAPT/PCP meetings. </li><li>Manages 9 week / quarterly report process. </li><li>Responds to stakeholder complaints regarding all aspects of treatment and care for client. </li></ul> <p>ADDITIONAL JOB FUNCTIONS</p> <ul> <li>Ability to provide Direct Care and/or supervision of clients is required </li><li>Ability to participate in limited the On-Call rotation </li><li>Managing hospitalizations both medical and acute </li><li>Ability to conduct preplacement interviews </li></ul> <p>MINIMUM TRAINING AND EXPERIENCE</p> <ul> <li>Master's degree in social work, psychology or counseling; or, </li><li>Bachelor's degree in social work or psychology with at least one year of documented field experience; or, </li><li>Bachelor's degree with at least three years experience working with children in a residential facility. </li></ul> <p>ADDITIONAL REQUIREMENTS</p> <ul> <li>Knowledge of CSA/Medicaid/Private funding processes. </li><li>Knowledge of services accessible to special needs/mental health population </li><li>Previous case management or like experience </li></ul> <p>SPECIAL REQUIREMENTS</p> <ul> <li>Requires a valid driver's license. </li><li>Flexible hours </li></ul> <p>COMPENSATION</p> <ul> <li>Based upon experience and credentials </li></ul> <p>EMPLOYEE BENEFITS</p> <ul> <li>Medical, dental and vision </li><li>Flexible Spending & Health Savings Accounts </li><li>401(k), including an employer match </li><li>Generous Paid Time Off plan </li><li>Education Assistance </li><li>Life Insurance </li><li>Employee Assistance Program (EAP) </li><li>Short-term disability (STD) </li><li>Long-term disability (LTD) </li><li>As Grafton is a 501(c)(3) non-profit, you may be eligible for federal student loan forgiveness </li></ul> <p>Grafton is an equal employment opportunity employer and tobacco-free workplace.</p> <p>#IND1</p>
