In fact, we're the only hospital in northwest Ohio to receive this honor.</p> <p>ORGANIZATIONAL RELATIONSHIPS</p> <p>Reports to pharmacy manager with no direct reports.</p> <p>POSITION SUMMARY</p> <p>Responsible for primary functions of a registered pharmacist: verifying prescription orders, counseling patients, interacting with physicians, and general oversight of pharmacy operations. Additionally, specialty pharmacists perform new start and clinical outreach calls, assist physicians with medication authorizations, and serve as patient resource to ensure high quality outcomes.</p> <p>ACCOUNTABILITIES</p> <ul> <li>Reviews prescriptions issued by physician, or other authorized prescriber, to ensure accuracy and appropriateness. </li><li>Compounds medications using standard formulas and proper aseptic technique. </li><li>Oversees pharmacy technicians engaged in preparing and packaging prescription orders. </li><li>Answers questions and provides information to pharmacy patients on drug interactions, adverse effects, dosage/storage/administration of medications, and other appropriate measures to ensure safe, effective use of medication. </li><li>Maintains established procedures concerning quality assurance, controlled substances (where applicable), and disposal of medications. </li><li>Enters data and maintains patient profile including patient name, prescribed medication, cost, insurance billing, and medication fulfillment. </li><li>Telephonic customer service working with patients, physicians, insurance companies, pharmaceutical manufacturers, patient assistance programs, and others as necessary. </li></ul> <p>Specialty Pharmacy Only</p> <ul> <li>Completing and submitting prior authorization forms with great attention to detail and accuracy. </li><li>Assisting prescribers with denial/appeal of reject claims. </li><li>Gather proper information and completing patient assistance applications. </li><li>Works proficiently within pharmacy and specialty workflow software systems. </li><li>Perform "New Patient Start" calls which include, but are not limited to: medication reconciliation, disease state evaluation, concomitant disease state evaluation, assess clinical appropriateness, and education/training. </li><li>Assess appropriateness of subsequent refills, address patient issues/concerns, monitor adherence, and create care plan when appropriate. </li><li>Shipping completed orders to patients and prescribers. </li><li>Participates in departmental performance improvement activities. </li><li>Participates in e-learning activities. </li><li>Maintains ongoing knowledge, understanding, and continuing education of related disease states and medications pertinent to specialty pharmacy. </li></ul> <p>Job Requirements</p> <p>REQUIRED QUALIFICATIONS</p> <p>Education: Bachelor's degree in Pharmacy or Pharm.D. degree</p> <p>Skills:</p> <p>Years of Experience:</p> <p>License: Must be a registered pharmacist, licensed in good standing in the state of practice or license eligible.</p> <p>Certification:</p> <p>PREFERRED QUALIFICATIONS</p> <p>Education:</p> <p>Skills: Comfort with pharmacy software systems and retail workflow</p> <p>Years of Experience:</p> <p>License:</p> <p>Certification: BLS - Basic Life Support</p> <p>ADDITIONAL EXPERIENCE</p> <p>WORKING CONDITIONS</p> <p>Personal Protective Equipment: Will wear appropriate PPE when compounding nonsterile, sterile, and hazardous products)</p> <p>Physical Demands: Extended periods of sitting at computer; daily standing during order fulfillment</p> <p>The above list of accountabilities is intended to describe the general nature and level of work performed by the incumbent; it should not be considered exhaustive.</p> <p>ProMedica is a mission-based, not-for-profit integrated healthcare organizational headquartered in Toledo, Ohio.
