<p>Summit Medical Group is recruiting for an RN Care Manager. This is a full time opportunity and after successful completion of introdcutory period this is a hybrid office/work from home position.</p> <p>This position provides individualized care to patients with complex, chronic medical conditions designed to ensure health care continuity for SMG patients.</p> <p>Examples of Duties (List does not include all duties assigned)</p> <ul> <li>Identify SMG patients with the potential for high-risk complications and coordinate the appropriate treatment in conjunction with the patient and Care Coordination team. </li><li>Develop and implement targeted strategies to improve overall patient health and quality of life outcomes. </li><li>Coordinate with SMG patient's primary care provider, specialists, community resources, families, and care programs, to ensure comprehensive, holistic, person-centered approach to care. </li><li>Provide patients with evidence-based education on chronic medical conditions. </li><li>Conduct chart reviews, educate patient, and facilitate follow up with provider to promote completion of age-appropriate preventive care procedures. </li><li>Establish an environment of open communication, using motivational interviewing techniques to enhance patient's motivation toward change. </li><li>Establish collaborative relationships with referral sources, community resources and care providers. </li><li>Act as an advocate for an individual's health care needs by identifying and communicating potential needs to the patient's provider. </li><li>Maintain security and privacy of patients by keeping patient care information confidential. </li><li>Participate in required training and attend team, departmental, and organizational meetings. </li><li>Monitor and improve quality of services provided to patients/caregivers through ongoing participation in team and departmental quality improvement activities. </li><li>Update job knowledge by participating in educational opportunities that support the advancement of Care Coordination. </li><li>Participate in the orientation and training of new staff as needed. </li><li>The RN Care Manager job description is not designed to cover or contain a comprehensive list of activities, duties, or responsibilities that are required of the </li></ul> <p>employee for this position. Each employee will contribute to the continual evaluation site performance as well as the implementation and measurement of improvement activities that increase the quality of care provided to patients. </li><li>Performs related work as assigned. </li></ul> <p>Education</p> <p>Associate or bachelor's degree in Nursing from an accredited college or university (required).</p> <p>Bachelor of Science in Nursing (Preferred).</p> <p>Experience</p> <p>Minimum 2 years of experience in clinical or administrative role in medical organization.</p> <p>Prior experience in care management, disease management, population health</p> <p>management, or other related healthcare environments, preferred.</p> <p>Certification/License</p> <ul> <li>Valid, active, and unencumbered State of Tennessee Registered Nurse License </li><li>Valid Driver's License. </li></ul>
