This position focuses on outreach, education, and support to ensure individuals can access the care and resources they need. By building relationships with individuals and organizations across Southeast Ohio and collaborating with university health services, the Community Health Worker plays a vital role in bridging healthcare access gaps. The role also involves collecting community feedback, assisting with resource navigation, and supporting initiatives to raise awareness and encourage utilization of Ohio University's health resources.</p> <p>Position Responsibilities:</p> <p>Community Outreach & Engagement</p> <ul> <li>Conduct in-person outreach in community spaces, health fairs, social service agencies, and public events to connect individuals with Ohio University's community health services. </li><li>Develop relationships with underserved populations to understand barriers to care and provide guidance on available health resources. </li><li>Represent Ohio University at community meetings and coalitions to promote awareness of campus-based health programs and establish collaborative partnerships. </li><li>Facilitate presentations and workshops to educate community members on healthcare services, prevention programs, and wellness initiatives. </li></ul> <p>Direct Support and Resource Navigation</p> <ul> <li>Assist individuals in accessing on-campus health services such as primary care, specialty care, mental health support, and preventative screenings. </li><li>Provide guidance on eligibility and enrollment for university-affiliated health programs, including free or reduced-cost services for qualifying individuals. </li><li>Help clients navigate healthcare systems by offering referrals, scheduling assistance, and follow-up support to ensure continuity of care. </li><li>Work with university clinics to streamline access to care for patients referred through outreach efforts. </li></ul> <p>Collaboration with University Health Services</p> <ul> <li>Work closely with the Heritage College of Osteopathic Medicine and the College of Health Sciences and Professions to align outreach efforts with university health initiatives. </li><li>Facilitate connections between community partners and university clinics to expand access and awareness of healthcare services. </li><li>Support interdisciplinary collaboration with faculty, students, and healthcare professionals to enhance community health engagement. </li><li>Identify opportunities for joint programming with local organizations to address regional health needs. </li></ul> <p>Addressing Barriers to Care</p> <ul> <li>Identify and address obstacles preventing access to Ohio University's health services in Southeast Ohio. </li><li>Develop strategies to improve awareness and utilization of university health resources for underserved and hard-to-reach populations. </li><li>Collaborate with stakeholders to advocate for more accessible and inclusive healthcare services. </li><li>Ensure that outreach and education efforts are clear, user-friendly, and responsive to the needs of the region. </li></ul> <p>Data Collection and Continuous Improvement</p> <ul> <li>Track outreach efforts and service linkages to assess program effectiveness and identify areas for improvement. </li><li>Collect feedback from community members to understand health needs and inform future outreach and engagement strategies. </li><li>Assist in developing reports and presentations summarizing the impact of outreach and resource navigation. </li><li>Stay informed on best practices by attending training sessions, networking with professionals, and engaging in professional development opportunities. </li></ul> <p>Minimum Qualifications</p> <ul> <li>Must either have completed, be currently enrolled, in or be willing to enroll in a Community Health Worker training course. </li><li>3 years of relevant professional experience. </li><li>Valid Ohio driver's license. </li><li>Ability to pass an FBI/BCI enhanced background check and the National Service Criminal History Check, in accordance with the organization's standards. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Certified Community Health Worker (C-CHW) certification. </li><li>Experience working in a healthcare or social service agency. </li></ul> <p>Department HCOM - Community Health Programs Pay Rate</p> <p>The approved hiring range for this position is $16.84 to $21.48 per hour.
