<p>Position Summary:</p> <p>In addition to performing the duties of the Specialized Mentor of Community Supports position, provides assistance to a Community Supports Supervisor in the general affairs of the program.</p> <p>Essential Functions: Include the following, other duties may be assigned</p> <p>Supports individuals living in the community in meeting their predetermined goals: may include personal care and hygiene, budgeting, shopping, cooking, meal planning, the use of community resources including public transportation, housekeeping and additional areas affecting the quality of life.</p> <p>Advocates for individuals, monitors and evaluates their progress with outcome-based results and prepares and submits appropriate reports.</p> <p>Participates and contributes to meetings inside and outside the agency as requested.</p> <p>Ensures safety and well-being of individuals served, including supporting those individuals in safety- related issues.</p> <p>Reports accidents and incidents involving individuals served, as required by MOKA policies and applicable regulations.</p> <p>Transports those served in personal/agency vehicles to appointments, shopping, social activities, etc. as per each individual's Person-Centered Plan.</p> <p>Investigates possibilities for social and recreational activities and encourages/supports and advocates for the participation of those served.</p> <p>Supports individuals in maintaining and developing their relationships.</p> <p>Supports those served in a personal way, offering friendliness without hindering their independent values and opinions.</p> <p>Interacts and works with individuals and others as part of a team to determine the needs of those served.</p> <p>Assesses individuals served at admission for appropriate employee assignments.</p> <p>Complete billing entries into billing software.</p> <p>Will become familiar with the hiring process and will be available to assist with interviews as needed.</p> <p>Conducts job specific orientation for new employees as assigned by the CS Supervisor and schedules and tracks training for new and existing staff.</p> <p>Maintains individuals served and staff records by ensuring proper filing of forms and documentation sheets.</p> <p>Maintains documentation of meetings and in-service training conducted in the CS Program.</p> <p>Completes staff scheduling and payroll as assigned by the CS Supervisor.</p> <p>Will perform on-call/cell phone coverage on a relief basis as assigned.</p> <p>Facilitates and encourages strong peer group support as a primary and ideal support.</p> <p>Identifies risk and behaviors that can lead to a crisis and uses effective strategies to prevent or intervene in the crisis in collaboration with others.</p> <p>Facilitates individuals learning their own behaviors that can lead to a crisis and strategies to manage those independently.</p> <p>Helps individuals manage tasks that are necessary on a day to day basis to pursue an independent community based lifestyle.</p> <p>Supports MOKA mission and works within the core values of the agency.</p> <p>All employees will respond to health risks or any hazardous condition(s) and communicate using the agency hierarchy any potential health risk, hazardous or unsafe condition(s).</p> <p>Qualifications</p> <p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.</p> <p>Education and/or Experience</p> <p>A Bachelor's degree in physical health or related field and at least one year experience working with individuals with developmental disabilities or equivalent. Prefer Certified Occupational Therapy Aid (COTA).</p> <p>Language Skills</p> <p>Ability to communicate in person, on the telephone and in written form to convey information at meetings, prepare reports, document the progress of those served and in case of accidents.</p> <p>Mathematical Skills</p> <p>Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.</p> <p>Reasoning Ability</p> <p>Must possess the ability to problem solve and make decisions in a variety of situations and direct others effectively in carrying out these determinations. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.</p> <p>Computer Skills</p> <p>To perform this job successfully, an individual should have knowledge of Database software; Internet software; Spreadsheet software and Word Processing software.</p> <p>Certificates, Licenses, Registrations</p> <p>Valid MI Driver's license and at least three years licensed driving experience.</p> <p>Other Qualifications</p> <p>Must have a personal mobile phone with willingness to make and receive calls on behalf of the agency as well as access to a personal computer with internet access for submission of required documentation. Must have a well maintained, insured vehicle to transport persons served. Must possess the ability to work independently and work cooperatively as part of a team. Must possess the ability to think creatively when reviewing existing procedures and proposing improvements as well as the ability to develop and implement creative strategies.</p> <p>Physical Demands</p> <p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee is frequently required to stand; walk; sit and use hands to finger, handle, or feel. The employee is occasionally required to reach with hands and arms; climb or balance and stoop, kneel, crouch, or crawl. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.</p> <p>Summa only: The employee is required to exert 50-120 pounds of force occasionally, (when lifting an individual served in cooperation with at least one other employee), or in excess of 50 pounds of force frequently, or 10-20 pounds of force constantly to move objects or assist clients. Physical demand requirements are in excess of those for medium work.</p> <p>Work Environment</p> <p>The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
