When hospitals cannot provide the advanced care, expertise and new treatment options their patients require, they turn to us.</p> <p>Job Summary:</p> <p>Responsible for providing clinical nutrition services, including nutrition assessment, modified diet formulation, self-management training and nutrition intervention to patients/residents. Ensures patient satisfaction, quality care, regulatory agency compliance, and good public relations are achieved through the safe and efficient use of resources.</p> <p>Key areas of responsibilities:</p> <ul> <li>Follows the Code of Ethics for the Nutrition & Dietetic Professional as established by the Academy of Nutrition & Dietetics (AND) and the Commission on Dietetic Registration (CDR) </li><li>Follows the guidelines according to the Nutrition Care Process as set forth by AND </li><li>Flexible schedule to meet the needs of departments (weekdays, weekends/evenings) </li><li>Able to travel to community locations as required for meetings and events </li><li>Serve as a lead in the development, implementation, and monitoring of nutrition-related educational plans for community members, outreach work plans, and other initiatives. </li><li>Responsible for the evaluation and ongoing assessment, and implementation of a comprehensive nutrition program. Involved in community outreach and educational events to increase overall service line knowledge and accessibility. </li><li>Development of nutritional materials for individuals and populations </li><li>Provides MNT (Medical Nutrition Therapy) to patients, specific to their diagnosis with consideration of physical, financial, social, emotional needs, cultural, and lifestyle </li><li>Implements health programs with an emphasis on disease prevention and health promotion to individuals and groups. </li><li>Participates in health fair activities in the local community. </li><li>Meets one-on-one with community members. </li><li>Develops and secures culturally appropriate informational and educational materials, guides, brochures, etc. </li><li>Successfully handles all duties essential to the efficient operation of the department </li></ul> <p>Bachelor's Degree in Food and Nutrition/Dietetics.</p> <ul> <li>Master's Degree in Nutrition or related field preferred. </li><li>One year of work experience in clinical nutrition preferred. </li><li>Registered Dietitian Nutritionist (Commission on Dietetic Registration);Must have Certified Dietitian-Nutritionist (CT Department of Public Health) within 90 days of hire. </li><li>*RD eligible (Must achieve Registered Dietitian credential and request CDN within 90 days of hire) </li></ul> <p>We take great care of careers.</p> <p>With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
