Specialists provide comprehensive clinical and distributive medication management services and will utilize their advanced clinical skills in the provision of evidenced-based, safe, and cost-effective drug therapy to achieve optimal medication therapy outcomes. They serve as a drug information and educational resource for the pharmacy staff, patients, residents/students, medical staff, nursing staff, and other healthcare professionals. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.</p> </li><li> <p>Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.</p> </li><li> <p>Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.</p> </li><li> <p>Provides drug information to physicians, nurses and other health care professionals.</p> </li><li> <p>Checks and verifies sterile and non-sterile medications.</p> </li><li> <p>Supervises the activities of students, technicians, and residents within the pharmacist area of practice.</p> </li><li> <p>Promotes a culture of patient safety by submittingmedication related events and near misses.</p> </li><li> <p>Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.</p> </li><li> <p>Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.</p> </li><li> <p>Precepts pharmacy students, resident rotations, andresident-led quality improvement projects. May serve as Residency Program Coordinator or Residency Program Director</p> </li><li> <p>Regularly participates in department initiatives including but not limited to supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources.</p> </li><li> <p>Supports a specific core area of practice through interdisciplinary education, ongoing specialty training and as a resource for Pharmacy staff.</p> </li><li> <p>Updates clinical guidelines and presents hospital wide educational initiatives.</p> </li><li> <p>Serves on hospital interdisciplinary committees or workgroups to improve process and quality outcomes.</p> </li><li> <p>Supports IRB-approved research activities, medication use evaluations, or cost-savings initiatives.</p> </li><li> <p>Other duties as assigned</p> </li></ol> <p>Required Qualifications:</p> <ol> <li>PGY 1 + 1-3 years OR PGY 2 OR 3-5 years of experience in clinical specialty area </li></ol> <p>Preferred Qualifications:</p> <ol> <li>BPS </li></ol> <p>As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
