Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care. Implement plan of care. </li><li>Communicate to Physicians, their assessment through surveillance exams, injuries, treatment and procedures as deemed necessary. </li><li>Develop individualized plan of care for all assigned patients.
