Employees can expect competitive pay and benefits.</p> <p>Summary:</p> <p>Provides chronic condition management and health coaching to members with disease risk indicators to achieve health risk reduction, improved quality outcomes, enhanced member experience, and cost-efficiency.</p> <ol> <li>Formal Education Required: </li></ol> <p>a. Bachelor's of Science in Nursing (BSN) or equivalent education and experience.</p> <p>b. Registered Nurse (RN) state license in good standing.</p> <ol start="2"> <li>Experience & Training Required: </li></ol> <p>a. A minimum of three years experience to include care coordination, chronic condition management, acute or managed care.</p> <ol start="3"> <li>Other Skills, Competencies and Qualifications: </li></ol> <p>a. Ability to perform tasks with little supervision</p> <p>b. Collect and analyze data from diverse sources; making recommendations and/or conclusions based on analyses</p> <p>c. Computer literacy to locate information from multiple sources, documentation, remote work, etc.</p> <p>d.
