<p>Oakwood Village has an exciting opportunity for qualified candidates to join our senior living community facility team as a Physical Therapist. This position directs and supervises the physical therapy programs for the facility. Responsibilities include evaluations, assessments, program planning, implementation, follow-up and case management to maximize each resident's ability to function independently in their activities of daily living. Leadership regarding that of Physical Therapist Assistants and Rehabilitation Aides.</p> <ul> <li>Per diem, with varied hours and shift </li></ul> <p>ESSENTIAL RESPONSIBLITIES (Not intended to be an all-inclusive list)</p> <ul> <li>Assess residents' needs; selects, administers and interprets appropriate evaluation procedures. </li><li>Supervises the planning and implementation of individual resident physical therapy programs. Assures treatment programs are modified as needed. </li><li>Works independently with little supervision and is sensitive to the needs of many constituents </li><li>Maintains appropriate documentation per regulatory guidelines. Prepares and submits all clinical billing in a timely manner </li><li>Provides leadership in further development/expansion of physical therapy services and projects to grow physical therapy programs for Oakwood </li><li>Meets the annual goals and objectives that are consistent with the values and mission of Oakwood Village and continuous quality improvement </li></ul> <p>ESSENTIAL QUALIFICATIONS</p> <ul> <li>Current WI licensure required. </li><li>Previous 1 year geriatric and/or subacute experience. </li><li>Minimum of one to two years of experience </li></ul>
