<p>The expected duration of this assignment is 6 weeks.</p> <p>Supports integrated operations and transitions of care activities. Coordinates patient placement with post-acute care facilities and/or services, transportation, and obtains insurance authorization to facilitate safe transitions of care.</p> <p>Performs diversified administrative support, coordinative and/or secretarial duties related to the functional and/or operational needs of department and Department Head.</p> <p>Responsibilities:</p> <ul> <li>Follows standard policies, techniques, and practices to facilitate safe transitions of care, including but not limited to post-acute placement, transportation, and or/appointments, and obtain appropriate insurance authorization for the next level of care. </li><li>Uses team-based communication strategies to close the loop on referrals, hospital follow ups, and any outstanding transition of care items identified in the patient's care plan. </li><li>Composes confidential correspondence, reports, memoranda, and related documents. </li><li>Schedules appointments and meetings. Makes necessary arrangements for meetings. Takes meeting minutes, as required. </li><li>Performs various receptionist and registration duties. </li><li>Compiles and analyzes basic information and prepares statistical data for reporting purposes. </li><li>Prepares presentation materials. Creates charts, graphs, tables, graphics, etc. </li><li>Maintains department supply and equipment inventory. </li><li>Performs research for special projects, as assigned. </li><li>Performs related duties, as required. </li></ul> <p>Qualifications:</p> <ul> <li> <p>High School Diploma or equivalent, required.</p> </li><li> <p>0-1 year administrative experience, required.</p> </li><li> <p>Working knowledge of Microsoft applications (Word, Excel, PowerPoint), required.</p> </li><li> <p>Ability to exercise independent judgment and employ basic reasoning skills.</p> </li><li> <p>Ability to communicate effectively.</p> </li><li> <p>Additional Salary Detail</p> </li></ul> <p>The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
