Home health provides the opportunity to work one-on-one with patients in their homes, helping them maintain or improve their health and manage their care.</p> <p>Responsibilities:</p> <p>Manage and coordinate care for an assigned caseload of patients</p> <p>Provide skilled nursing care using evidence-based practice and critical thinking</p> <p>Collaborate with interdisciplinary team members, patients, and families to promote continuity of care</p> <p>Supervise and direct support staff in the provision of care</p> <p>Deliver safe, age-appropriate nursing care focused on optimal patient outcomes</p> <p>Minimum knowledge, skills, and abilities:</p> <ul> <li>Graduate of an accredited RN program </li><li>Current registration and licensure in the State of Iowa as an RN. Must be in good standing with the Board of Nursing. </li><li>Current American Heart Association (AHA) Heart Code BLS required. New employees certify within three (3) months after hire date. </li><li>Nursing experience in Home Health or hospital setting preferred. </li><li>Must possess a valid Iowa driver's license and meet satisfactory motor vehicle record requirements. </li><li>Must maintain and provide proof of his/her own auto insurance policy. </li><li>Ability to read and communicate effectively in English. </li><li>High degree of empathetic listening skills, tact and kindness toward others. </li><li>Ability to communicate well verbally. Must work effectively as a member of a team. </li><li>Basic computer knowledge </li><li>Technical and critical thinking skills in relation to sound nursing judgment. </li></ul>
