<p>Summary of Position: Assumes responsibility for the overall nutrition management of patients.# Uses The Nutrition Care Process to assess patients, identify patient-focused nutrition prescriptions and interventions and monitor and evaluate as appropriate.# Re-assesses patients based on level of nutrition care complexity and Dept. policy. Works with healthcare team to determine required diet and patient education needs.# Provides evidence-based nutrition education to patients, families, staff and the community.# Interfaces with Diet Office and tray passing personnel.# Performs daily patient rounds; appropriately screens patients based on established guidelines and Dept. policy.# Maintains complete and appropriate records.# Performs other duties as required. Excellent communication and interpersonal skills. Commitment to excellent patient care required.# Must be able to use a personal computer and calculator. Assumes responsibility for maintaining current knowledge related to the age and development of the patient populations served by this position. Ability to be analytical and self-motivated, ability to prioritize tasks and work effectively as part of a team. Minimum Qualifications: Bachelor of Science degree in Human Nutrition and Dietetics.# Must be a Registered Dietitian, with current registration through The Academy of Nutrition and Dietetics#(AND). CDN Certification required.
