Provides case management including continuum care planning and advocacy. Serves as department community resource development specialist and as a liaison between select patients of all ages and community service systems. Case Management. </li></ol> </li></ul> <p>Qualifications</p> <p>EDUCATION</p> <p>Bachelor of Social Work degree from a program accredited by the Council on Social Work Education or equivalent bachelor's degree with experience in working in a related helping profession or with community agencies.</p> <p>EXPERIENCE</p> <p>May be a new graduate. Preference is given if experienced in health or mental health social work or related helping profession.</p> <p>SPECIAL SKILLS</p> <p>Competency in caring for patients treated at the Hospital regardless of age, disability, sexual preference, gender, ethnicity, or diagnosis.
